    Music in the first days of life

    In adults, specific neural systems with right-hemispheric weighting are necessary to process pitch, melody and harmony, as well as structure and meaning emerging from musical sequences. To which extent does this neural specialization result from exposure to music or from neurobiological predispositions? We used fMRI to measure brain activity in 1 to 3 days old newborns while listening to Western tonal music, and to the same excerpts altered, so as to include tonal violations or dissonance. Music caused predominant right hemisphere activations in primary and higher-order auditory cortex. For altered music, activations were seen in the left inferior frontal cortex and limbic structures. Thus, the newborn's brain is able to plenty receive music and to figure out even small perceptual and structural differences in the music sequences. This neural architecture present at birth provides us the potential to process basic and complex aspects of music, a uniquely human capacity

    Drug-Related Hypersensitivity Reactions Leading to Emergency Department: Original Data and Systematic Review

    The aim of the present study is to describe pharmacological characteristics of drug-related allergies and anaphylaxis leading to the emergency department (ED). An 8-year post hoc analysis on the MEREAFaPS Study database was performed (2012–2019). Subjects who experienced drug-related hypersensitivity leading to an ED visit were selected. Logistic regression analyses were used to estimate the reporting odds ratios (RORs) of drug-related allergies and anaphylaxis adjusting for sex, age classes, and ethnicity. In addition, a systematic review of observational studies evaluating drug-related hypersensitivity reactions leading to ED visits in outpatients was performed. Out of 94,073 ED visits, 14.4% cases were drug-related allergies and 0.6% were anaphylaxis. Females accounted for 56%. Multivariate logistic regression showed a higher risk of drug-related allergy among males and all age classes < 65 years, while a higher risk of anaphylaxis was observed for females (ROR 1.20 [1.01–1.42]) and adults (ROR 2.63 [2.21–3.14]). The systematic review included 37 studies. ED visits related to allergy and anaphylaxis ranged from 0.004% to 88%, and drug-related allergies and anaphylaxis ranged from 0.007% to 88%. Both in our analysis and in primary studies, antibacterials, analgesics, and radiocontrast agents were identified as the most common triggers of hypersensitivity

    Risk of hospitalization for adverse drug events in women and men: A post hoc analysis of an active pharmacovigilance study in italian emergency departments

    This post hoc analysis of an Italian active pharmacovigilance study describes pharmacological differences of ADEs leading to emergency department (ED) visits and hospitalization in women and men. During the study period (January 2007–December 2018), 61,855 reports of ADEs leading to ED visits were collected. Overall, 30.6% of ADEs resulted in hospitalization (30% in women and 31% in men). Multivariate logistic regression showed that, among women, drug classes significantly associated with an increased risk of hospitalization were heparins (ROR 1.41, CI 1.13–176), antidepressants (ROR 1.12, CI 1.03–1.23) and antidiabetics (ROR 1.13, CI 1.02–1.24). Among men, only vitamin K antagonists (ROR 1.28, CI 1.09–1.50), opioids (ROR 1.30, CI 1.06–1.60) and digitalis glycosides (ROR 1.32, CI 1.09–1.59) were associated with a higher risk of hospitalization. Overall, older age, multiple suspected drugs and the presence of comorbidities were significantly associated with a higher risk of hospitalization. A significantly reduced risk of hospitalization was observed in both women and men experiencing an adverse event following immunization (ROR 0.36, CI 0.27–0.48 and 0.83, 0.42–0.74, respectively) compared to drugs. Results obtained from this real-world analysis highlight important aspects of drug safety between sexes

    WHATSAPP MESSENGER AS A REAL-TIME TOOL FOR A LONG-DISTANCE ACTIVITY OF A MULTIDISCIPLINARY

    Introduction: Communication between doctors is traditionally conducted by written clinical charts. Mobile health is becoming an integral part of modern medical systems, improving accessibility and quality of medical care. Recent papers suggest that an increasing number of doctors are using in their clinical practice mobile tools to communicate clinical informations (1, 2). The aim of our study was to verify the adoption of WhatsApp Messenger in everyday clinical practice to obtain a real-time multidisciplinary collaboration among medical centers located in different areas of the city. Materials and Methods: In January 2016 a WhatsApp Messenger group was created among 25 specialists: 9 urologists, 9 oncologists, 3 urology residents, 3 radiotherapists and 1 general practitioner. A general coordinator and a group coordinator for each specialty was monthly appointed. The participants were invited to interact within the group clinical cases of genitourinary tumors of particular complexity requiring a multidisciplinary approach. All the chats were registered. A preliminary analysis of the activity of the group was planned after the first 10 entered patients. An evaluation questionnaire was sent after 6 months to evaluate the level of appreciation. The questionnaire was composed of a first section investigating the appreciation among the members of the group and a second section analyzing the impact in their everyday clinical practice of whatsapp multidisciplinary consultation. Results: In 10 (91%) out of 11 patients the WhatsApp consultation was completed, one case was not of oncological interest. An average of 8 (range=2-13) specialists joined the chat for each patient. An average of 17.6 (range: 4-43) interventions for each clinical case was recorded. On the average, 27%, 54% and 19% of the interventions for each clinical case were provided by oncologists, urologists and radiotherapists respectively. In 9 (81.8%) cases a final agreement on the patient's management was reached. At the evaluation questionnaire in a scale 1-10, the average rating score of appreciation was 7.8 (range=4-10). Relevant suggestions to improve the Whatsapp Messenger consultation were obtained and will be considered for future application the ameliorate the tool. Discussion: WhatsApp is a useful alternative and powerful complementary communication tool because of its capability to rapidly transfer large amount of clinical and radiological data. In our experience this new approach for multidisciplinary consultations improved collaboration among different specialist in different areas of the city through an easier and more informal change of opinions. In difficult and complex cases a rapid multidisciplinary approach allowed to offer the patient a personalized and tailored therapy management.     Integrated Operational Taxonomic Units (IOTUs) in Echolocating Bats: A Bridge between Molecular and Traditional Taxonomy

    Background: Nowadays, molecular techniques are widespread tools for the identification of biological entities. However, until very few years ago, their application to taxonomy provoked intense debates between traditional and molecular taxonomists. To prevent every kind of disagreement, it is essential to standardize taxonomic definitions. Along these lines, we introduced the concept of Integrated Operational Taxonomic Unit (IOTU). IOTUs come from the concept of Operational Taxonomic Unit (OTU) and paralleled the Molecular Operational Taxonomic Unit (MOTU). The latter is largely used as a standard in many molecular-based works (even if not always explicitly formalized). However, while MOTUs are assigned solely on molecular variation criteria, IOTUs are identified from patterns of molecular variation that are supported by at least one more taxonomic characteristic. Methodology/Principal Findings: We tested the use of IOTUs on the widest DNA barcoding dataset of Italian echolocating bats species ever assembled (i.e. 31 species, 209 samples). We identified 31 molecular entities, 26 of which corresponded to the morphologically assigned species, two MOTUs and three IOTUs. Interestingly, we found three IOTUs in Myotis nattereri, one of which is a newly described lineage found only in central and southern Italy. In addition, we found a level of molecular variability within four vespertilionid species deserving further analyses. According to our scheme two of them (i.e. M. bechsteinii and Plecotus auritus) should be ranked as unconfirmed candidate species (UCS). Conclusions/Significance: From a systematic point of view, IOTUs are more informative than the general concept of OTUs and the more recent MOTUs. According to information content, IOTUs are closer to species, although it is important to underline that IOTUs are not species. Overall, the use of a more precise panel of taxonomic entities increases the clarity in the systematic field and has the potential to fill the gaps between modern and traditional taxonomy

    Improved language recognition using better phonetic decoders and fusion with MFCC and SDC features

    Proceedings of Interspeech 2007, Antwerp (Belgium)One of the most popular and better performing approaches to language recognition (LR) is Parallel Phonetic Recognition followed by Language Modeling (PPRLM). In this paper we report several improvements in our PPRLM system that allowed us to move from an Equal Error Rate (EER) of over 15% to less than 8% on NIST LR Evaluation 2005 data still using a standard PPRLM system. The most successful improvement was the retraining of the phonetic decoders on larger and more appropriate corpora. We have also developed a new system based on Support Vector Machines (SVMs) that uses as features both Mel Frequency Cepstral Coefficients (MFCCs) and Shifted Delta Cepstra (SDC). This new SVM system alone gives an EER of 10.5% on NIST LRE 2005 data. Fusing our PPRLM system and the new SVM system we achieve an EER of 5.43% on NIST LRE 2005 data, a relative reduction of almost 66% from our baseline system.This work was funded by the Spanish Ministry of Science and Technology under project TEC2006-13170-C02-01

    Acoustic Event Recognition for Low Cost Language Identification

    Actas de las V Jornadas en Tecnología del Habla (JTH 2008)One of the most popular approaches to Automatic Language Identification (LID) is Parallel Phone Recognition followed by Language Modeling (Parallel PRLM or PPRLM). This approach has proved to be very sucessful in LID. However, it has two mayor drawbacks: its high computational cost duw to the need to run several phone recognizers on the same test segment; and the need to train the phone recognizers on manually transcribed data that may not match closely the type of speech on which the system will work. In this paper we present a novel approach for LID that tries to solve these two problems. It is based on substituting the phonetic recognizers by an Acoustic Event Recognizer (AER) that can be trained on untranscribed data and is much faster than the phone recognizers. Results show that this method, which we call AERLM, can be much faster than PRLM, although at the cost of reduced LID precision, and therefore suitable for low-cost LID

    EEG Hyperconnectivity Study on Saxophone Quartet Playing in Ensemble

    A professional quartet of saxophonists playing in ensemble provides a perfect scenario to study the eventual occurrence of synchronous oscillatory brain activity across subjects. Here, we applied hyperscanning methodologies for simultaneously recordings of electroencephalographic (EEG) signals from four professional saxophonists while they observ an audiovideo recording of their own previous musical performance. An ad-hoc musical composition was written for the study. At debriefing, the subjects were asked to answer two questionnaires to assess their empathy trait and the musical leadership. In order to estimate the hyperconnectivity of each musician we proposed a measure which combines phase synchronization index of brain oscillations and graph theory framework. The inter-connectivity level of each musician was statistically compared. Statistical results revealed a significant lower hyperconnectivity in the left Brodmann area 44 for the Soprano with respect to the other three members. Recent theories attributed this brain region (Broca's area) to music generation, empathy processes and communication. We hypothesize a relationship between brain-to-brain connectivity level and the musical role within the quartet
